As a client, you keep your own medical team, and continuing disease-focused treatment does not affect your eligibility. We meet you where you are.
Our team of will work together to develop a plan of care tailored to your individual needs and wishes. Our services are designed to help you regain some control at a time when life can seem out of control and allow you and your family to focus on the things that matter most to you.
We do not limit our support to clients—all of our resources are available to the community at no cost. Anyone is welcome to stop by our office during open hours to access:
DME Closet – Durable medical equipment and supplies available for short‑term or long‑term use.
Library – Books and materials to help navigate death, dying, caregiving, and grief.
End‑of‑Life Planning – Access to advance directive forms, power of attorney documents, Five Wishes, legacy project materials, and funeral planning tools. We can answer your questions and walk you through how to complete these documents.
Dementia‑Friendly Items – Education, materials, and supportive items for individuals living with dementia and their caregivers.
We connect individuals to trusted community partners and resources when needs fall outside our services to help you find the support you need.
Whether you need medical hospice, palliative care, senior housing support, mental health services, or something else entirely, we’ll help point you in the right direction and make sure you’re not navigating the system alone.
We do this to the best of our ability, and while we may not always have a specific resource available, we will still support you in exploring next steps, so you won’t have to navigate this alone.
We offer regular, open grief groups, monthly classes, and seasonal programs to support navigating loss. Please see our calendar for the current or upcoming group and events.
